For reference, your <br />original comments are in italics. <br />APPLICATION FORM: <br />1) It appears that there is a discrepancy in the acreage portion of the application form (item <br />#3). Prior to the approval ofAM-03 the permit acreage was 176.24 acres. After AM -03 was <br />approved the permit area was increased to 177.34 acres. Based on the Division's review of <br />AM -02, it appears that 0.327 acres of land will be added to the approved permit, bring the <br />total permit acreage to 177.67. Please re -submit the first page of the amendment application <br />with the correct acreage. <br />Attached is the revised Page 1 of the application form showing the acreage of 177.67. <br />EXHIBIT E — Reclamation Plan (Rule 6.4.5): <br />2) The seed mix in AM -02 is different than the seed mix described in AM -01. Both seed mixes <br />are enclosed. Please clarity which seed mix will be used in final reclamation. <br />The seed mix will be the same as AM -01. <br />3) The Division has reviewed the pre -mining groundwater data submitted with the original <br />permit dating back to 1980. The Division specially look at the groundwater elevation along <br />the course of the french drain from A to A'. The pre -mining groundwater elevation at A was <br />approximately 5007 feet while at A' the groundwater elevation was approximately 5005 feet <br />(enclosed groundwater contour map). This is an approximate 2 -foot elevation change over <br />the course of where the proposed french drain would be, from A to A'. The design proposes <br />setting the french drain elevation at 5010 at A feet and 5007 feet at A'. The proposed french <br />7405 W.
