QUARTERLY COAL WASTE BANK INSPECTION REPORT <br />CDMR Rules 4.09.1(11) and 4.10.2(2) <br />Mine: New Elk Mine <br />Waste Bank MSHA ID. No.:_ _Development Waste Disposal Area #1 <br />Inspection Period: First Quarter 2015 <br />Inspection Date: <br />March 25. 2015 <br />General Description or Reference to Site Plan: <br />The Refuse Disposal Area is located south of State Highway 12 and west of the main Mine Access Road. <br />It is adjacent to the north of the Middle Fork of the Purgatoire River. The area has been used for disposal <br />of waste rock from the underground mine. Since mine closure in 1989, the area has been in the process of <br />being reclaimed and is currently in Phase 1 of Bond released. <br />Activity during Inspection: <br />The DWDA# 1 is in the second stages of reclamation with no activity occurring or scheduled to happen in <br />this area, except for maintenance should erosion be a problem. <br />Drainage: <br />No springs or seeps were observed on the refuse side slopes. Positive drainage is established on all areas. <br />Vegetation is well established on previously reclaimed areas, with little to no erosion. <br />Observation of Fill Construction: <br />Fill construction has been completed. Reclamation of the surface has been completed. <br />Indications of Potential Failure or Instability: <br />None observed. Surface and out slopes were inspected for potential failure. <br />Threat to Human Life or Property: <br />None observed. Location and placement minimalize any potential impacts to life or property. <br />Potential Harm to Land, Air, and Resources: <br />None observed. Location of the area minimizes any potential impacts to the land and water resources. <br />Maintenance Required: <br />No additional work required at the time of this inspection. <br />Supporting Documentation: <br />None required at this time. <br />i <br />3595 <br />nspector _ ` MSHA Id # Sup Lvryising E. 1 <br />
