This amendment application is <br />submitted in response to the Division's request that TR -04 be withdrawn and re -submitted as an <br />amendment. As the basis for requiring the amendment was a change proposed for reclamation of one area <br />of the operation, I believe that you indicated that we should submit with the application form only those <br />exhibits which have changed since the previous amendment, together with a list indicating which exhibits <br />have not changed from the previous amendment. Therefore, included with this application is an original <br />and four (4) copies of each of the following: <br />Regular 112 Operation Reclamation Application Form <br />Amendment Application AM -002 Exhibits (list) <br />Exhibit E — Reclamation (narrative) <br />Exhibit F — Reclamation Plan Map <br />Exhibit L — Reclamation Costs <br />Exhibit Q — Proof Of Mailing Of Notices To County Commissioners And Soil Conservation District <br />Exhibit R — Proof Of Filing With County Clerk Or Recorder <br />Addendum I — Notice Requirements Certification <br />Filing Fee - Check No. 194600 in the amount $1,750.00 <br />The fling fee check submitted herewith has been reduced by $188.00, as we have been advised that the <br />fee previously submitted with our technical revision request (TR -04) would be applied to the amendment <br />application fee. Enclosed is a copy of the Division's March 4, 2002 letter verifying this. <br />We appreciate your assistance in this process.
