PERMIT #: M-1980-244 <br />INSPECTOR’S INITIALS: AME <br />INSPECTION DATE: August 12, 2015 <br /> <br /> <br />Page 6 of 15 <br />Mine Area. <br /> <br />Post-Inspection Meeting: <br />After the inspection was completed, the inspector met with Mr. Campbell and Mr. Hanks at the operator’s <br />headquarters in Victor. The discussion included the following topics: <br /> <br /> A wash-out that was observed on the northern side of AGVLF Phase V (where driplines blew off their <br />headers; Photo 14) which mine staff are working to repair. This incident is not uncommon as the <br />AGVLF matures, and any spilled cyanide-solution was completely contained on the geomembrane lined <br />area of the leach pad. <br /> <br /> How the AGVLF water levels are being maintained below 80% capacity. <br /> <br /> The continued use of misters on the AGVLF to help manage the overall water balance while the <br />operator works to bring additional areas on Phase IV under leach to redistribute flows (further <br />decreasing flows to Phases I and IV). <br /> <br /> The approval of Technical Revision No. TR-74 (on 07/10/2015) for a modification of the SGVLF toe <br />berm underdrain pumpback system. Installation activities associated with this revision should begin <br />shortly. The operator committed to providing the Division with as-constructed information for the <br />concrete sump lining activities and as-built pumpback and associated pipeline locations as part of the <br />SGVLF Phase I CQA certification report. <br /> <br /> Introduction to the new General Manager and Environmental Manager (since Newmont Mining <br />Corporation’s recent acquisition of CC&V Gold Mining Company). <br /> <br />This concluded the inspection. <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br />
