PERMIT #: M-1977-342 <br />INSPECTOR'S INITIALS: PSH <br />INSPECTION DATE: August 21, 2015 <br />OBSERVATIONS <br />The Henderson Mill facility was inspected by Peter Hays and Stephanie Mitchell with the Division of <br />Reclamation, Mining and Safety (Division/DRMS) as part of the Division's monitoring inspection program. Mr. <br />Tim Haynes with Climax Molybdenum - Henderson Operations (Henderson) was present during the inspection. <br />The drilling and well construction for the Ute Park Extraction Wellfield — Phase 2 project was ongoing during <br />the inspection. Extraction well MLEX-2 was completed at the time of the inspection. The drilling of extraction <br />well MLEX-3 was complete and completion of the well was observed during the inspection. Well MLEX-3 was <br />drilled to 75 feet below grade. The Troublesome Formation was located at 55 feet below grade. The <br />extraction well was screened from 56 to 25 feet below grade. The well was expected to be completed the <br />next day. The drilling of MLGW-23/24 was scheduled to be next followed by MLGW-25 near the TSF sand seep <br />project. The pad for MLGW-23/24 and the pitless adapters for MLEX-2 and MLEX-3 were observed during the <br />inspection. <br />The TSF sand seep liner system project was ongoing and was observed during the inspection. The sand seep <br />area excavation and backfilling with compacted low permeability fill was complete. The installation of the <br />geotextile fabric and geomembrane were also completed. The toe anchor trench for the geomembrane was <br />backfilled. The top and side anchor trenches were open and observed during the inspection. Evidence of <br />geomembrane seam testing and backfill compaction testing were observed. The process of covering the <br />geomembrane with 12 inches of select native material was scheduled to begin soon. <br />Noxious weed control spraying occurred the week of the inspection according the Henderson staff.
