PERMIT #:M-1981-158 <br />INSPECTOR'S INITIALS:LJW <br />INSPECTION DATE:August 10, 201:; <br />UBSERVA11UNS <br />I his inspection was conducted as part of the normal monitoring program established by the Luluradu Division <br />of Rcclamatiun, Mining and safety. Furest CaKes Pit Ido. 1 is a 11Uc operation located approximately seven <br />miles Forth of Bayfield, Colorado. The site lies bChind a fire statiuii un county Road 5U1, access is controlled <br />by a IuLKed gate. I Re site consists of b.34 permitted acres with an $11,688.00 financial warranty. ruur phutus <br />accompany this r cpurt to illustrate current site cunditiuns. <br />The permit area is lucated within a 4U acre parcel owned By the Forest Lakes Metro District. i he parcel <br />contains various mun;cipal i esourcCs including a wastewater treatment tacility and tire station. The site was <br />not active at the time of the inspection, But stockpiles of excavated materials were observed un site. I Re pit <br />area serves only tRe Metro District tur ruad base, and Bedding materials for new utility lines within the district. <br />Proper signage was ubscrvCd at the Entrance to the site; Ruwever it was taded to the point where it was <br />almost unreadaBle. The operator agreed to replace the sign and potentially move it closer to tRe entrance to <br />the site. Buundary markers were also observed, however the markers along the South permit boundary, <br />shown in Phots Onc, had been removed accidentally. 1 he uperatur also agreed to replace the tew missing <br />Boundary markers along the Southern Boundary. This is not being cltCd as a pi ublem at this time. <br />The site currently employs cuntcmpui aneous reJaination in filling ut tRe pit tram the south as excavation <br />takes place on the Northern end of the pit. The pit is currently filled with water, and is subject to seasunal <br />filling as explained by the uperatur. I Re appropriate water augmentation plan is in place to account for the <br />exposed water. Excavation and crushing uf materials takes place as needed witR as contracted company to <br />Lai ry uut the crusRing, with the material being stockpiled on site and used continuously fur various utility <br />projects in the ivictru District. <br />i he operatur discussed tRe claritication of a previously discussed issue, specitically in the 2011 inspection <br />report by Divisiun staff. i hC matter has been clarified and the uperatur needs to suBmit an attidavit for the <br />importation of ottsite till material, to maintain compliance with Rule 3.1.5 (`J). 1 he uperatur expressed that <br />they uccasiunally impurt till material generated By La Plata County during road clean up uf a ncarby <br />reoccurring land slide. Providing this affidavit will allow fur tRe importdtivn of the till material and is not Being <br />cited as a proBlem at this time. <br />The overall footprint of tha site was in gaud, Londitiun, witR nu excess trash and deBris associated with the <br />mine on site. Evidence of chemical treatment of noxious weeds was observed as seen in PRoto Four. no <br />Crusiun ur sedimentation prublems were observed on site. The Division will Be recalculating the rCclamatiun <br />Bond according to the current site conditions as uf this inspCLtiun. I he bund calculation will Be sent to the <br />uperatur under a separate cover. <br />Responses to this inspection report should be direLtcd to [upas west at the Divisiun Lit Reclamation, Mining <br />and Safety, 1313 Sherman Street, Denver, CU SUZU3. <br />Page 2 of 5 <br />
