Box 18087 <br />Boulder, CO 80308 <br />RE: Goose Haven Reservoir #2; DRMS File No. M-2010-071; Receipt of 112 Construction <br />Materials Reclamation Permit Amendment Application Package <br />Dear Mr. Wayland, <br />On August 5, 2015, the Division of Reclamation, Mining and Safety (Division) received your <br />112 Construction Materials Reclamation Permit Amendment Application package for the Goose <br />Haven Reservoir #2, File No. M-2010-071. Preliminary review of the application package <br />determined that the following items must be received before the Division can consider your <br />application as being submitted and technical review can begin: <br />6.4.17 Exhibit Q — Proof of Mailing of Notices to Board of County Commissioners and Soil <br />Conservation District <br />The Applicant submitted copies of the Certified mail card to demonstrate proof of mailing of <br />Notices to the Board of County Commissioners and the Soil Conservation District. However, <br />the tracking numbers associated with each of the Certified mailings showed that the Notices <br />have not been delivered. As required by Rule 1.6.2(1)(a)(ii), proof of notice shall be in the <br />form of a return receipt from a Certified mailing or a date-stamped copy of the notice <br />acknowledging receipt by the appropriate local Board. Please provide the Division with <br />copies of the return receipts from the Certified mailings. <br />Your application will not be considered submitted until the information listed above is received <br />and found sufficient to begin our review. A decision date will be established 90 days from the date <br />of receipt of all of the requested information. Additionally, if you have already published notice <br />you will need to republish notice, after the Division considers the application submitted. This <br />notice must be published once a week for four (4) consecutive weeks, starting within ten (10) days <br />of the date your application is considered submitted. We will notify you when you should initiate <br />republication of your notice.
