Review of this <br />application determined the following items must be received before the Division can consider the application as <br />being submitted and the technical review can begin. Please respond to this letter with the requested information on <br />or following a cover letter titled "Response to Incompleteness CN -01" and include the permit number specified <br />above. <br />APPLICATION FORM <br />1. Within the General Operation Information section, on the first page of the application, the question numbered <br />5 regarding what are the primary commoditie(s) to be mined was left blank. Please correct this and re -submit <br />Page 1 of the application. <br />2. Within the General Operation Information section, on the first page of the application, the question numbered <br />5.1 regarding what are (if any) the incidental commoditie(s) to be mined and associated quantities per year <br />was left blank. Please correct this and re -submit Page 1 of the application. <br />3. The Division believes the Primary Mine Entrance Location provided on the application question 410 is <br />incorrect. Based on a Google Earth image, the location appears to actually be 2,250 feet west of the <br />coordinates provided. Please provide a new application page 2 with the correct coordinates or provide proof <br />that the coordinates listed are correct. <br />4. Within the Responsibilities as a Permittee section, on the sixth page of the application, the statement <br />numbered 10 was initialed. The Division believes this was a mistake and actually needs to be left blank. <br />Please correct this and re -submit Page 6 of the application <br />EXHIBIT A — Leual Discription (Rule 6.4.1): <br />The legal description provided contains a typo.
