PERMIT #: M-2006-031 <br />INSPECTOR'S INITIALS: MAC <br />INSPECTION DATE: November 21, 2014 <br />OBSERVATIONS <br />The inspection was conducted by Michael Cunningham of the Division of Reclamation, Mining and Safety <br />(Division) and Amy Titterington of the U.S. Forest Service (USFS). Mr. Scott West was present on behalf of <br />Steel Rose Inc. The Steel Rose 1 site is located approximately 13 miles northwest of Lake George and 2 miles <br />north of State Highway 24 on Forest Road 226 in the Pike -San Isabel National Forest. The site is permitted for <br />9.0 acres and the post -mining land use is rangeland. <br />The inspection was conducted as part of the Division's regular monitoring program. On July 18, 2014, the <br />Division approved Amendment No. 2 which increased the permit area from 3.39 acres to 9.0 acres. The permit <br />sign was posted at the site entrance and the permit boundary was clearly delineated with monuments. The <br />site was not active at the time of the inspection; no equipment was present. According to the Operator, the <br />site was active in the past month. The Operator continues to mine within the existing trench, which is <br />approximately 300 feet in length and 40 feet in width, and 25 feet deep. The Operator has stockpiled waste <br />rock around the perimeter of the site; there is approximately 11,000 cubic yards of overburden stockpiled. The <br />topsoil has been salvaged and there are stockpiles on both side of the trench. <br />The Operator informed the Division that mining will continue to occur at the south end of the trench. The <br />reclamation work planned for 2015 includes backfilling the northern portion of the trench. The Operator is <br />encouraged to time the reclamation work such that replacement of topsoil will coincide with either a spring or <br />fall seeding. Additional care should be taken control annual weeds on the site when reclamation work is <br />complete. The Division has not observed any state listed noxious weeds at the site. <br />The Division currently holds a financial warranty in the amount of $12,600. It is estimated that the Operator <br />has affected approximately 4 acres of the 9.0 acre permit area. The financial warranty is adequate to reclaim <br />the existing disturbance at the site. <br />This concluded the inspection. <br />Page 2 of 3 <br />
