PERMIT #: M-1986-007 <br />INSPECTOR'S INITIALS: ECS <br />INSPECTION DATE: July 22, 2015 <br />OBSERVATIONS <br />This inspection was conducted by Eric Scott and Wally Erickson of DRMS for the Masonville Stone site as a <br />follow up to the April 2015 inspection at this site. DRMS representatives met with Glenn Southwick, and <br />Michael Lang (VP — Coffey Engineering and Surveying) at the site at approximately 0945 hours. The site <br />entrance is located on the south side of Larimer County 29 RD approximately 100 feet west of the intersection <br />of 29 RD and 27 RD. The required postings were present at the site entrance. This site is currently an active <br />110c sandstone/flagstone quarry. The active mining at the site at the time of the inspection was occurring <br />near the center of the site. <br />This follow up inspection was conducted to meet with the permittee and his surveyor in another attempt to <br />resolve the ongoing boundary issue noted in the April 23, 2015 inspection report for this site. A survey of the <br />disturbed area submitted by the permittee to DRMS on June 30, 2015 as a corrective action did not appear to <br />correspond well to the current disturbed area. To resolve this discrepancy, DRMS, the permittee, and the <br />surveyor all walked and staked the perimeter of the existing disturbed area as well as the un -impacted "seed <br />bank" set-aside area located within the mined area. The resulting survey data will establish the total affected <br />area to date, and determine what corrective permitting actions will be required. DRMS has assigned a <br />corrective action due date of August 7, 2015 for the submittal of the acreage totals and a certified map of the <br />existing disturbance. <br />The other issues noted in the April 2015 inspection report, insufficient reclamation bond, and missing permit <br />boundary markers will still need to be resolved as appropriate when the total disturbed acreage has been <br />determined. DRMS again strongly advises the permittee to submit a 110c permit amendment or 112c <br />conversion application so that sufficient additional acreage can be added to the existing 9.4 acre permit to <br />avoid further boundary issues in the future. <br />As stated on the second page of this report, it was noted during this inspection that there was a significant <br />amount of knapweed present on site. This is a listed noxious species and will need to be eliminated from the <br />permit area. Control measures will need to be implemented and demonstrated to DRMS by August 31, 2015, <br />and a weed control plan will need to be submitted as a technical revision to the existing permit, or included as <br />an attachment with any pending permit amendment or conversion application. <br />No other serious problems were noted during the inspection. DRMS discussed the site, the problems noted, <br />and corrective action options with the operator. This concluded the inspection and DRMS left the site at <br />approximately 1430 hours. <br />Page 3 of 4 <br />
