Baker <br />COLORADO <br />Division of Reclamation, <br />Mining and Safety <br />Department of Natural Resources <br />1313 Sherman Street, Room 215 <br />Denver, CO 80203 <br />Loloff Construction, Inc. <br />8018th Street, Suite 130 <br />Greeley, CO 80631 <br />Re: Loloff Construction, Inc., Loloff Mine, File No. M-1985-112, <br />Complaint Against Mining Operation (CT -04), Updated Problems and Corrective <br />Actions <br />Mr. Baker, <br />The Loloff Mine was inspected by Peter Hays with the Division of Reclamation, Mining and <br />Safety (Division/DRMS) on July 11, 2015 in response to a citizen complaint (CT -04) received on <br />May 27, 2015 from Mr. Clarence Lopez on behalf of Marcos Hernandez and Maria de Lourdes <br />Palma. The Hernandez property, 160 N. 1st Avenue, Greeley, CO 80631, is located northwest of <br />the Loloff Mine. <br />Following the inspection the following problem requiring corrective actions was cited: <br />INSPECTION TOPIC. Hydrologic Balance <br />PROBLEM/POSSIBLE VIOLATION: Problem: The dewatering activity at the Loloff Mine <br />has caused disturbances to the prevailing hydrologic balance of the surrounding area <br />and to the quantity of water in the groundwater system. <br />CORRECTIVE ACTIONS: Loloff Construction, Inc. must implement the approved <br />mitigation plan for the site and drill a new well, deepening the existing well or provide <br />water service from the City of Greeley or North Weld Water within 30 days of the mailing <br />date of this inspection report. Failure to comply with the corrective action will result in <br />the problem escalating to possible violations and an enforcement hearing being <br />scheduled in front of the Mined Land Reclamation Board. <br />CORRECTIVE ACTION DUE DATE: 7/15/15 <br />The Operator failed to complete the corrective actions by the corrective action due date. On <br />July 7, 2015, Mr. J.C.
