PERMIT #: M-1977-301 <br />INSPECTOR'S INITIALS: MAC <br />INSPECTION DATE: June 18, 2015 <br />Phase IV: <br />Active quarry operations are occurring in Phase IV. Phase IV consists of a small pit approximately 1 acre in size <br />located on the south side of the access road near the top of the ridge. This pit is only quarried on an as needed <br />basis. The second active area of Phase IV is located north of the access road. Drilling and blasting has created <br />a highwall along the northern extent of the quarry. The Operator has salvaged and stockpiled topsoil on top of <br />the highwall. Overburden in this area has been stockpiled to the south of the quarry, so that it may be pushed <br />north (downhill) during reclamation. <br />Phase V• <br />No disturbance was noted within Phase V. <br />A review of the permit file found that the Division does not have copies of the Mining Plan or Reclamation <br />Plan Maps which conform to the requirements of Construction Material Rules 6.2.1(2), 6.4.3, and 6.4.6. <br />Specifically, the maps which are on file do not have a clearly labeled permit boundary. A new set of maps will <br />need to be submitted through a Technical Revision. Additionally, the Division believes there may be a <br />discrepancy in the permit file in regard to how many acres are permitted for this operation. Division records <br />indicate the permit area is 360 acres in size. Past inspection reports, annual reports and documents from the <br />Operator reference a 360 acre permit boundary. However, the original application was submitted for 90 acres. <br />The Operator may provide documentation which clarifies the size of the permitted area. Alternately, a new <br />permit boundary will need to be established and/or clarified through a Technical Revision. This has been cited <br />as a problem and will require corrective action by the Operator. Additional details regarding the required <br />corrective action may be found on the first page of this report. <br />As noted above, the Division holds a financial warranty in the amount of $565,500.00. The Division will <br />perform a complete bond calculation as a part of this inspection. Notice of the Division's decision regarding <br />the surety reduction request will be sent under separate cover. <br />This concluded the inspection. <br />Page 3 of 5 <br />
