The report was certified by a professional engineer in accordance with Rule 4.05.9(14) and <br />contains all of the required information in accordance with Rule 4.05.9(15). Also, this report appears to <br />be in compliance with Rule 4.05.9(17). The report included inspection report forms for each of the <br />ponds inspected. However, it does not appear a report form for the 6MN Reservoir was submitted. Was <br />this impoundment inspected? If so, please submit an inspection report. <br />It was reported that the ponds were inspected on June 29, 2015. No appearances of structural weaknesses <br />or hazardous conditions were reported. The report indicated several maintenance activities were <br />conducted.
