This report is prepared to comply
with the Environmental Protection Plan approved by the Division of Reclamation, Mining, and
Safety ( "DRMS ") and the conditions set forth in Attachment B of the Bureau of Land
Management's Decision Record, Finding of No Significant Impact, and Final Environmental
Assessment for the Whirlwind Mine Uranium Mining Project, September 2008.
During the first quarter of 2014, Energy Fuels Field Personnel made two attempts to access the
Mine. Due to the county road conditions, the Mine was inaccessible during the quarter, and
therefore, the quarterly water level /flow measurements and field parameters from DP Spring and
PR Spring could not be collected. Energy Fuels will conduct routine monitoring in the second
quarter of 2014, and the results will be reported to DRMS on or before July 30, 2014.
If you have any questions or comments, please do not hesitate to contact me at 303 - 389 -4167.
Yours very truly,
ENERGY FUEL, SOURCES (USA) INC.
Jaime Massey
Regulatory Compliance Specialist
cc: Scott Gerwe, BLM
Frank Filas
Kathy Weinel
Race Fisher
Andrea Reither
