Box 1660 <br />Cortez, CO 81321 <br />Phone 970-565-8000 <br />Fax 970-565-0188 <br />Web www.skanska.com <br />RE: Cortez Pit, Permit M-1976-059, Annual Fee Invoice and Report Request <br />To Whom It May Concern, <br />For the previous permitted year of June 2014 - June 2015 Skanska did not perform any mining, <br />processing, or hauling operations at the Cortez Pit (M-1976-059). Activities were limited to <br />maintenance of slopes and erosion control measures. No major erosion was observed on or near the <br />area of operation. Permit boundary signs are intact and the overall condition of the site was <br />acceptable. Please find an attached the completed annual report, a site map and a check for an annual <br />fee of $791.00. Our prior designee is no longer with Skanska, so updated contact information has <br />been indicated on the Annual Report. <br />We offer the following as clarification to one of the answers on the Annual Report: <br />To my knowledge, there is not an approved Weed Control Plan for this pit. However, we do inspect <br />the site for noxious weeds as part of our Stormwater Permit inspections. As of my inspection on June <br />9, 2015, there were some Canadian Thistle or Bull Thistle weeds observed in a limited area of the <br />site. Per our internal weed control plan, Weedmaster herbicide (sample label attached), will be <br />applied per manufacturer's recommendations; with a follow-up inspection to verify elimination <br />within a month.
