PERMIT #: M-2004-017 <br />INSPECTOR'S INITIALS: PSH <br />INSPECTION DATE: May 8, 2015 <br />inspection report. <br />No active storm water permit on file <br />The complaint letter states no active storm water permit is on file for the site. Mr. Willits provided evidence <br />of an active storm water permit for the site and provided a copy of an updated storm water permit for the <br />Division's file during the inspection. <br />Storage of material and equipment not associated with the mining operation <br />The complaint letter states equipment associated with the ATH Excavating business is stored at the site. The <br />Division does not regulate the storage of material and equipment not associated with the mining operation. <br />This is the responsibility of the County government. Mr. Gray with Grand County stated the equipment <br />storage was appropriate for the County permit. <br />The Division currently holds a financial warranty of $57,100, which appears adequate to complete the <br />reclamation of the site based on the observations from this inspection. An entrance sign was posted and <br />permit boundary markers were observed as required by Rule 3.1.12. <br />The land containing the mine site was recently purchased by Mr. Todd Hammerlund. Mr. Hammerlund and <br />Mr. Willits are planning to transfer the reclamation permit to Mr. Hammerlund, who will continue to mine the <br />site and operate his excavation business from the site. <br />Photographs taken during the inspection are attached. <br />Page 4 of 6 <br />
