OPERATORS <br />SHOULD READ THE FOLLOWING PAGES CAREFULLY IN ORDER TO ASSURE COMPLIANCE <br />WITH THE TERMS OF THE PERMIT AND APPLICABLE RULES AND REGULATIONS. If a <br />Possible Violation is indicated, you will be notified under separate cover as to when the Mined Land <br />Reclamation Board will consider possible enforcement action. <br />INSPECTION TOPIC: Backfilling & Grading <br />PROBLEM/POSSIBLE VIOLATION: Problem: The approved Mine Plan requires the Operator to maintain a 25 <br />foot offset from the property line and mine the perimeter at the final 3HAV slope. The northeastern highwall <br />has been mined to within ten feet of the permit boundary and exceeds the approved perimeter mined slope of <br />3HAV. <br />CORRECTIVE ACTIONS: The Division will require the Operator to backfill the highwall to establish the required <br />25 foot offset from the eastern boundary and the required 3HAV slope and provide evidence within 60 days of <br />the mailing date of this report. <br />CORRECTIVE ACTION DUE DATE: 7/25/15 <br />INSPECTION TOPIC: Hydrologic Balance / Wash Pond <br />PROBLEM/POSSIBLE VIOLATION: Problem: A wash pond containing water is located in the northeast corner of <br />the pit floor. The Division was not able to determine if the source of the water in the former wash pond was <br />groundwater or trapped stormwater. <br />CORRECTIVE ACTIONS: The Division will require the Operator to backfill the pond to 2 feet above the water <br />surface and provide evidence within 60 days of the mailing date of this inspection report. <br />CORRECTIVE ACTION DUE DATE: 7/25/15 <br />Page 1 of 6 <br />
