May 26, 2015 C-2009-087/Peabody Sage Creek Mine JLE <br />SLIDES and DAMAGE - Rule 4.12: <br />A slide appears to have occurred above the uppermost portal entry on the southeast side of the portal area. PSCM <br />should repair and stabilize this area. <br />SUPPORT FACILITIES - Rule 4.04: <br />The mine office, parking area, shop area, and ventilation system/storage/substation benches above the portal area <br />were observed and no problems were noted. <br />TOPSOIL — Rule 4.06 <br />Removal 4.06.2; Substitute Materials 4.06.4(4); Storage and Protection 4.06.3; Redistribution 4.06.4: <br />Topsoil Pile A is vegetated and stable. <br />OTHER: <br />The maintenance items listed below were observed during the April 7, 2015 inspection; it is unclear if these items <br />have been addressed. The Division will evaluate these items during the next inspection. <br />• The berm surrounding the coal storage area and water tank has eroded and is cutting into the CH -2 <br />channel and will need to be repaired. During this inspection it was noted that Channel CH -2 is eroding <br />downstream from the berm. This channel drains to the upper sump. The channel embankment has eroded <br />through and needs to be repaired. <br />• Near the inlet to the Lower Sump from drainage FA -D8 where the drainage turns to drain into the sump, <br />the west side of the drainage is eroding and cutting into the slope above. PSCM will need to stabilize this <br />area. <br />• The weir at Pond 003 is leaking and will need to be repaired. <br />• Some hounds tongue was observed on Topsoil Pile A and will need to be treated. <br />DOCUMENTS RECEIVED; NA <br />OTHER (SPECIFY): NA <br />Number of Partial Inspection this Fiscal Year: 9 <br />Number of Complete Inspections this Fiscal Year: 3 <br />Page 3 of 10 <br />
