Each of the 10 evaluated records contained all of the information indicated above with the <br />following exceptions: <br />• 5/18/2015 (This was the first record employing CCC's new revised blasting record form): <br />Record included the distance to the nearest dwelling but omitted the direction. Direction <br />could be discerned from other locational information provided but should be indicated on <br />the record. Form asks for "direction to nearest structure" so this was a minor omission <br />likely due to the use of a new / revised form. Delay detonator type was not clear. The <br />listed blaster -in -charge name and license number differed from the blaster -in -charge <br />signature. One blaster -in -charge should be listed by name, license number, and signature. <br />• 5/14/2015: The listed blaster -in -charge name and license number differed from the <br />blaster -in -charge signature. <br />• 5/11/2015: The listed blaster -in -charge name and license number differed from the <br />blaster -in -charge signature. <br />• 5/7/2015: The listed blaster -in -charge name and license number differed from the blaster - <br />in -charge signature. <br />• 5/6/2015: The listed blaster -in -charge name and license number differed from the blaster - <br />in -charge signature. <br />• 5/4/2015: Did not list the delay detonator type (delay periods were listed). One record <br />(5/18/2015) utilized this new form. Due to our findings regarding the delay detonator type and <br />blaster -in -charge identification noted above, we worked with Chip Brown to alter the new form. <br />Mr. Brown added lines to the form to indicate Blaster Leadman (lead person in the field during <br />blast activities) name and license number as well as the overall Blaster -in -Charge name, license <br />number, and signature. He also added language to clearly indicate delay detonator type on the <br />same line as delay times used. <br />When identifying the type of material blasted, CCC indicates whether it was overburden or coal <br />and labels the appropriate strata, e.g. Overburden G7. Many other coal mine operators will label <br />overburden type specifically such as sandstone, siltstone, shale, etc. This practice is beneficial in <br />the event a particular blast requires computer modeling to re-create, for instance if a nearby <br />landowner alleged that a blast damaged their property. While this would be more protective of <br />the mine operator, it is not an explicit programmatic requirement. <br />CCC employs a computer program that identifies the maximum number of holes that will <br />4 I p a a <br />
