(blue earth <br />SOLUTIONS <br />May 28, 2015 <br />Division of Reclamation, Mining and Safety <br />1313 Sherman Street, Room 215 <br />Denver, Colorado 80203 <br />Subject: 25th Avenue Site, M-2002-020, Annual Report <br />This annual report summarizes mining and reclamation activities at the 25th Avenue Site, Permit <br />number M-2002-020, during the report year ending May 30, 2015. The following attachments are <br />included with this cover letter. <br />1. Annual fee check for $791.00, <br />2. Completed annual report form, <br />3. Site ground water monitoring data, and <br />4. Annual report map. <br />Mining and reclamation highlights from the report year include: <br />➢ The mining cell in the southwest corner of the property, approximately 30 acres in size, is <br />currently being dewatered and mined. Existing side slopes have been graded to 3H:1V or <br />flatter and 3H: IV side slopes are being maintained as mining progresses. <br />➢ A 75 -foot width on the south side of the mining cell was backfilled and graded with 3H: IV <br />slopes to make room for future slurry wall development. <br />➢ Non -marketable materials from the mining cell are temporarily stockpiled at the edge of the <br />SE Pond and then used to backfill the pond as part of final reclamation. <br />➢ The permit area north of the Cache la Poudre River (approximately 40 acres) remains <br />undisturbed. <br />For additional information please contact Blue Earth Solutions or the City of Greeley Water and <br />Sewer Department. <br />Sincerely, <br />Blue Earth Solutions, LLC <br />/t il f <br />J1&- I <br />W////i////lliam Schenderlein <br />Attachments <br />Blue Earth Solutions, LLC * P.O.
