PERMIT #: P-2014-005 <br />INSPECTOR'S INITIALS: MAC <br />INSPECTION DATE: June 1, 2015 <br />OBSERVATIONS <br />The inspection was conducted by Michael Cunningham of the Division of Reclamation, Mining and Safety <br />(Division). No representative from Maisie Mining Company was present for the inspection. The Capital Prize <br />Mine is located at 1016 Biddle Street on the west side of Georgetown, Colorado. The Capital Prize Mine is an <br />historic mine that was founded in the late 1800's. <br />On May 4, 2015, the Division received a request for a full performance and financial warranty release. The <br />release request was sent in accordance with Hard Rock Rule 4.16. The purpose of the inspection was to <br />determine if reclamation had been completed in accordance with the approved reclamation plan and the <br />Reclamation Performance Standards outlined in the Hard Rock Rules. Prospecting at the Capital Prize Mine <br />was to consist of bulk sampling and there was to be no surface disturbance associated with the project. <br />According to the Operator, a dispute over the lease agreement with the landowner prevented prospecting <br />activities from occurring. In addition, Maisie Mining Company did not obtain authorization from the Town of <br />Georgetown to proceed with the proposed project. <br />The Division confirmed that no additional surface disturbance had occurred as a result of the proposed <br />prospecting project. The underground workings of the Capital Prize Mine were not inspected. The mine portal <br />has a locked gate preventing unauthorized entry into the mine. Based on the observations made during this <br />inspection, the site is eligible for a full release from reclamation responsibility. Notice of the Division's decision <br />regarding the release request will be sent under separate cover. <br />This concluded the inspection. <br />Page 2 of 3 <br />
