Mr. Bergmann <br />Page 3 <br />May 26, 2015 <br />6.3.4 Exhibit D — Reclamation Plan <br />11. As required by Rule 6.3.4(2), please provide an estimate of the actual costs to reclaim the site <br />based on what it would cost the State of Colorado using an independent contractor to <br />complete reclamation. The unit costs should include estimates for the following activities: <br />backfilling and grading. Unit costs (cost per cubic yard), volumes, haul or push distances, <br />and grades must be included when backfilling and grading are part of the Reclamation Plan. <br />12. The Mining Plan states underground openings will be permanently sealed. Please specify <br />how the adits will be permanently sealed. <br />13. The bond calculation from the U.S. Forest Service (USFS) accounts for closure of one adit. <br />Please verify this is the most recent bond calculation from the USFS. <br />14. The Reclamation Plan states the waste rock dumps will be covered with tarps if deemed <br />necessary by the USFS. Please confirm that placement of tarps over the waste rock pile <br />would be a temporary measure to be employed during active mining operations and that all <br />tarps would be removed from the site during final reclamation. <br />15. The POO calls for grading the site such that no rough piles higher than 18" would remain. <br />However, the Reclamation Plan states no rough piles higher than 24" would remain. Please <br />clarify this discrepancy. <br />16. Please clarify if the gate at the lower end of the access road will be removed during final <br />reclamation of the site. <br />6.3.5 Exhibit E — Map <br />17. Please revise the Mining Plan Map to include the following: <br />a. A labeled permit boundary. The permit boundary shall correspond to the proposed 2.: <br />acre permit boundary. <br />b. Label the names of owner(s) of record of the surface of the affected area and of the <br />land within 200 feet of the affected area and identify the owner of the substance to be <br />mined. <br />c. Outline and label all existing and proposed adits. <br />d. Show the name of the applicant. <br />e. Give date prepared. <br />f. Map must be signed by a registered land surveyor, professional engineer, or other <br />qualified person. <br />g. For ease of use the Division requests the applicant change the units on the scale bar <br />from metric to standard. <br />
