Havengar <br />y..,Lazy H Inc <br />34449 County Road 10 <br />Keenesburg, CO 80643 <br />Re: Lazy H Inc, Bootleg Quarry, File No. M-2015-022, Receipt of 111 Special Operations <br />Construction Materials Reclamation Permit Application Package- Incomplete [„_-eA- <br />Mr. Havengar, <br />On April 28, 2015, the Division of Reclamation, Mining and Safety (Division/DRMS) received the <br />111 Special Operations Permit Application package for the Bootleg Quarry, File No. M-2015-022. <br />Preliminary review of the information received determined the following items must be received <br />before the Division can consider the application as being submitted and technical review can <br />begin: <br />Application Form <br />1. Please revise Item 49 — Quarter/Quarter Section selection to "SW" to match the legal <br />description provided in Exhibit A. <br />2. The Division requires corporations to affix a corporate seal on the certification page (Page 8) <br />of the Construction Materials 111 Special Operation Reclamation Permit Application Form. <br />Please submit a revised, original copy of the certification page with a corporate seal. If Lazy H <br />Inc does not have a seal, write "no seal" in the space provided. <br />3. The Division requires the Applicant to provide documentation giving the signer, William K. <br />Havengar, the authority to sign on behalf of the company. The authorization document may <br />be in the form of by-laws, resolution, affidavit signed by corporate secretary, certified <br />secretary statement or a notarized statement on company letterhead. <br />6.3.7 Exhibit G - Source of Legal Right to Enter <br />4. The Division did not receive Exhibit G-1. Please provide a description of the basis for legal <br />right of entry to the site and to conduct mining and reclamation. This may be a copy of access <br />lease, deed, abstract of title, or a current tax receipt.
