PERMIT #: M-1977-342 <br />INSPECTOR'S INITIALS: PSH <br />INSPECTION DATE: April 29, 2015 <br />The Baseline Parameter Report stated the NPL for Manganese at the Mill at 0.79. The NPL for the Mill <br />should be 0.42 as stated and approved in TR -16 — Groundwater Management Plan. <br />The testing at the Aspen Canyon Ranch well for establishing NPLs for Manganese and Iron were being <br />conducted the week of this inspection. The test results and report are expected to be completed in the <br />near future. The feasibility of installing a new well at Aspen Canyon Ranch may also be investigated by <br />Henderson. <br />• The acceptance of the POC well locations proposed in the 5 -Quarter Water Quality Data and Baseline <br />Parameters Report was not address in the April 14, 2015 memo. The Division will issue a memo <br />clarifying the POC wells proposed in the 5 -Quarter Water Quality Data and Baseline Parameters Report <br />are acceptable. <br />Henderson staff will be submitting two new technical revisions for the Henderson permit in the near future. A <br />technical revision for Phase 2 of the Ute Park Extraction Wellfield and for a Tailing Storage Facility (TSF) Liner <br />System for a sand seam identified on the north end of the TSF above the current tailings elevation.
