Signs and Markers (Rule 4.02) <br />The mine identification sign at the entrance from County Road 65 is intact and <br />contained all necessary information in accordance with 4.02.2 (operator name, address, <br />telephone number, and permit number displayed at all entrances to the site from public <br />roads). <br />All observed topsoil stockpiles were marked in accordance with 4.02.7. <br />Sediment Control Measures (Rule 4.45.5) <br />3 1 P a g e <br />cement. <br />LWB 11 -2 <br />Reclaimed <br />Reclaimed in 2013, casing below grade so could not verify within 36" of <br />surface), needs monument. Mr. Wanstedt indicated this casing may be well <br />outside the 36" surface proximity and need extending plus monument <br />before bond release. <br />LWB 12 -3 <br />Reclaimed <br />Surface stable, ripped, and seeded last fall. Casing below grade, needs <br />monument. Very little vegetative establishment. Relying on surface <br />roughness for erosion control. No erosional features noted. <br />LWB 13 -4 <br />Active <br />Mr. Wanstedt opened pressure valve, no mine gas present, fence <br />materials on site but not installed. This pad exhibits a 9% grade, as <br />measured by manual inclinometer, putting it just outside of the 8% max <br />grade used in SEDCAD demonstrations to support surface roughening <br />as sediment control. Observed adjacent 4" nitrogen injection line. Topsoil <br />stockpile properly marked. <br />LWB 13 -5 <br />Active <br />Light rain. Borehole fenced off, surface stable, ripped, and seeded but <br />barren within 25' radius of borehole as required by MSHA. Signage <br />indicating "danger," "hazardous gas," and "no smoking." Small perimeter <br />berm as sediment control to prevent OSIs. Topsoil stockpile properly <br />marked. <br />LWB 12 -5 <br />Reclaimed <br />Ripped, roughened, and seeded in fall, 2014. Missing monument. <br />LWB 10 -4 <br />Reclaimed <br />Reclaimed approx. 2011. Casing below grade, no monument. Ripped, <br />roughened, and seeded. Stable surface. <br />LWB 9 -3 <br />Reclaimed <br />Reclaimed approx. 2007, good grass establishment, surface stable. <br />LWB 13 -3 <br />Active <br />Fenced off, observed mine gas leak at flange, it was apparent PUF had <br />been used in a past attempt to stop the leak. Surface roughened and <br />seeded, very little veg present, no sediment deposition observed. <br />Extensive tumble mustard. <br />LWB 13 -2 <br />Active <br />No mine gas present upon valve opening. Fenced off. Topsoil salvage <br />pile very small, minimal vegetation, proper signage. Topsoil cover at the <br />entire site is very thin with some salinity issues. Observed red - tailed <br />hawk, a chipmunk, and elk scat. <br />LWB 13 -1 <br />Active <br />Fenced off. Surface stable, ripped, and seeded. Minimal vegetation. <br />Topsoil pile very small, properly marked.
