May 4, 2015 C- 1991 -081 CCW <br />Comments <br />This was a complete inspection conducted by Clayton Wein and Janet Binns of CDRMS. The <br />Hayden Gulch Load out (HGLO) consists of a partially reclaimed train load out /load facility, an <br />unreclamed rail loop, substation, two sediment ponds, and an unreclaimed haul road (the Tie - <br />Across Haul Road). There has been no recent activity on the HGLO. There are no mine <br />personnel on -site at the HGLO. No mine personnel accompanied the inspection. <br />Conditions at the site were damp but not muddy at the time of the inspection. There were no <br />livestock being held at the reclaimed facilities pasture. <br />The reclaimed load out site and Tie - Across Haul Road were inspected on 4/27/2015. Records <br />were verified at the Peabody Sage Creek Mine office on 4/29/2015. <br />AVAILABILITY OF RECORDS: Records were verified at the Peabody Sage Creek Mine <br />office, which is located at the Peabody Sage Creek Mine site. The records form accompanies <br />this report. <br />HYDROLOGIC BALANCE — Rule 4.05 <br />Drainage control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures <br />4.05.7, 4.05.10; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18 <br />The truck loop pond and rail loop pond are located at the load out facilities area. The <br />truck loop pond was not discharging at the time of the inspection, nor did the pond appear to <br />have weeds. The rail loop pond was not discharging at the time of the inspection and only had a <br />small pool of water. There are no concerns noted with either of these two ponds. <br />All culverts under the rail loop were clear and flowing freely. <br />The majority of the culverts under the haul road were clear; however, there are a few culverts <br />that need to be noted for maintenance. <br />C -10: The inlet of the culvert is crumpled in a bit. <br />C -13: The inlet is starting to side -cut a bit. (See Photo 1) <br />C -18: The inlet needs to be cleaned out and fixed. (See Photo 2) <br />C -19: Has sediment that needs to be cleaned out. <br />ROADS — Rule 4.03 <br />Drainage 4.03.1(7), 4.03.2(7): <br />The Tie - Across Haul Road (TAHR) was well maintained and graded. Runoff from the <br />haul road was directed off of the road appropriately, and there was no ponding of water <br />observed. The road was damp but not soft of muddy. The roadside ditches were functioning as <br />designed. As noted above in this report four of the culverts under the TAHR need to be <br />maintained. The TAHR was in good condition during the inspection. <br />Number of Partial Inspections this Fiscal year: 6 <br />Number of Complete Inspections this Fiscal Year: 4 <br />
