PERMIT #: M- 2001 -114 <br />INSPECTOR'S INITIALS: ECS <br />INSPECTION DATE: April 23, 2015 <br />OBSERVATIONS <br />This was a routine monitoring inspection conducted by Eric Scott of DRMS of the Loukonen Bros. Stone — <br />Starks Quarry site. This quarry is adjacent to and contiguous with the Weaver Quarry (M- 1986 -085) which is <br />another 110c sandstone /flagstone quarry owned and operated by Loukonen Brothers Stone. The division <br />between these two permits runs through the center of the'mining area and for all intents and purposes this <br />could be viewed as a single site. <br />DRMS representative Eric Scott met with Loukonen Brothers representative Michael Loukonen at the site at `' <br />approximately 1100. The site is located off the north side of Larimer County Road 27 as the road bends east to <br />enter Masonville. The required signs were present at the site entrance. <br />While on -site DRMS collected GPS data to confirm the placement of the NW permit corner as currently <br />marked. This marker appears to be misplaced based on the permit map on file with DRMS (see attached <br />map). The SE permit corner was also located with GPS for confirmation; however the location of that marker <br />(large wooden post) appears to match well with the map on file. The issue of missing /misplaced permit <br />boundary has been noted as a problem on the first page of the report, and a corrective action due date of May <br />29, 2015 has been set to resurvey /replace the missing and misplaced markers and provide DRMS with <br />evidence that this has been done. If it is determined as a result of the survey and replacement of the permit <br />boundary markers based on the legal description of the site, that.the DRMS'map is incorrect, the permittee <br />should submit a new permit boundary map to DRMS for inclusion in the site file. <br />The permittee should also be mindful of the proximity of the quarrying operation to the blacktop road that <br />runs parallel to the west boundary of the site. It would be the permittee's responsibility to repair any damage <br />to this road that occurs as a result of instability caused by mining activity. <br />It was apparent during the inspection that the two adjacent 110c sites were being mined and operated as a <br />single entity, which could result in possible permitting violations. DRMS would be willing to meet with the <br />operator to attempt to resolve these issues through a permit amendment or other appropriate permitting <br />mechanism. A good first step would be to re- establish all boundaries so that it is clearly identifiable where all <br />permit boundaries are located. DRMS strongly suggests that the permittee consider the conversion of one of <br />the two existing 110c permits to a 112c permit that incorporates the entire operation. <br />This site was operating at the time of the visit and appeared to be in compliance with the approved mining <br />plan at the time of the inspection. All disturbances were within the permitted area and no compliance issues <br />were noted at this time.
