Reclamation Permit Application Package
Dear Mr. Bergmann,
On April 15, 2015, the Division of Reclamation, Mining and Safety (Division) received your
Hard Rock Limited Impact 110(1) Reclamation Permit Application package for the Sara Rose,
File No. M- 2015 -020. Preliminary review of the application package determined that the
following items must be received before the Division can consider your application as being
submitted and technical review can begin:
Application Form
1. The `Responsibilities as a Permittee' section of the Application Form (Page 5 and 6) was not
initialed by the Applicant. Please compete and return this portion (copy enclosed) of the
Application Form.
6.3.10 Exhibit J — Proof of Mailing of Notices to Board of County Commissioners and Soil
Conservation District
2. The Applicant did not provide proof that notice of the application was sent to the Board of
Supervisors of the local Soil Conservation District. Please submit proof that notice of the
application was sent to the Soil Conservation District, proof of notice may be in the form of a
return receipt of a Certified Mailing or a date - stamped copy of the notice acknowledging
receipt by the appropriate local Board.
Your application will not be considered filed until the information listed above is received and
found sufficient to begin our review. A decision date will be established 30 days from the date of
receipt of all of the requested information. Additionally, if you have already published notice you
will need to republish notice, after the Division considers the application submitted. Republication
must occur once within ten (10) days of the submittal date. We will notify you when you should
