According to our records, <br />the following problems were to be resolved on or before July 25, 2014. Listed below are descriptions and <br />corrective actions for the problems. Please resolve the corrective actions and provide evidence of such <br />resolution on or before May 29, 2015. The Operator may also provide the Division with an update of the <br />status of each corrective action along with a request to extend the corrective action date. <br />Please note that if the corrective action(s) are not properly addressed, and evidence provided to the <br />Division by the date provided above, the listed problems could be escalated to the level of possible <br />violations and would then be referred to the Mined Land Reclamation Board for further action. <br />1. Problem: Failure to obtain a structure agreement for a permanent man -made structure (electric line) <br />within 200 feet of the permit boundary as required by Rule 6.3.12. <br />Corrective Action: The Operator shall obtain a structure agreement with the owner of the utility on <br />or before the corrective action date or shall provide an appropriate engineering evaluation that <br />demonstrates that such structure shall not be damaged by activities occurring at the mining operation. <br />If the problems have already been remedied, please provide evidence of their completion.
