PERMIT #: M- 2004 -078 <br />INSPECTOR'S INITIALS: ECS <br />INSPECTION DATE: April 21, 2015 <br />OBSERVATIONS <br />This was a routine monitoring inspection for the Lupton Lakes site, a 112c sand and gravel quarry permitted by <br />Denver Water and operated by Martin Marietta Materials (MMM). The inspection was conducted by Eric Scott <br />of DRMS. DRMS met onsite With Greg Gully of Denver Water, and Julie Mikulas of MMM and accompanied <br />them on a tour of the site. <br />Mining activity appeared to be in compliance with the approved plan at the time of the inspection, and the site <br />appears to be well run and maintained. The only issue noted during this inspection was the omission of permit <br />boundary markers from several areas of the permit (See attached map). These areas have been unaffected by <br />mining activity and are located outside the fence that delineates the rest of permit boundary. In fact, one of <br />these areas, located on the east side of WCR 27, has recently become the site of a new commercial structure <br />completely unrelated to the mine site. The area south of WCR 6 is still an agricultural field, and the third area is <br />property not owned by Denver Water and is the location of a gas and oil well. <br />As stated on the first page of this inspection report, DRMS will require that these currently permitted areas <br />either be properly marked and maintained as stated in the approved mining and reclamation plans, or a release <br />request submitted to DRMS to remove them from the permitted area by the corrective action date (May 29, <br />2015). Failure to do so may result in enforcement action for failure to follow the approved mining and <br />reclamation plan.
