PERMIT #: M- 1992 -038 <br />INSPECTOR'S INITIALS: TOD <br />INSPECTION DATE: February 10, 2015 <br />OBSERVATIONS <br />This normal routine monitoring inspection was conducted by Tyler O'Donnell of the Division of Reclamation, <br />Mining and Safety (DRMS). Daren Palizzi represented Jones Fine Sand Company, the Operator, during the <br />inspection. <br />The Sand Creek Mine is located in Adams County approximately 7.25 miles north- northwest of Byers <br />Colorado. The Sand Creek Mine is a 45.5 -acre 112c Construction Materials Reclamation Permit with a <br />maximum allowed disturbance of 10 acres at any given time. The primary commodity beginning mined at the <br />site is sand. The permit was issued in 1992. The approved post - mining land use is rangeland. <br />During the inspection, the sky was clear and the ground was dry. No mining equipment was active during the <br />inspection. The Operator had a bulldozer and a front -end load in the pit. It appears that the pit sees frequent <br />use. The Operator appears to be removing material from stockpiles located along the southwestern portion of <br />the affected area and mining the southwestern pit westward. At the present time it appears that the mining <br />operation has affected approximately 9 to 10-acres. <br />Backfilling and Grading_ <br />Some backfilling and grading has occurred along the southern end of the affected area. Slopes in the affected <br />area ranged from 1.5H:1V to flatter. The Operator shall regrade all portions of the pit slopes that are steeper <br />than 4H:1 V, to 4:1V or flatter, to comply with the approved reclamation plan. The pit contains approximately <br />1,000 linear feet of highwalls with an average highwall height of approximately 15 feet. The highwalls run <br />along portions of the western and southern affected land boundaries. There were several stockpiles of product <br />scattered throughout the site. <br />Financial Warranty: <br />The current amount of financial warranty the Operator has on deposit with the State is $7,210.00. The <br />reclamation cost estimate for this site was updated with this inspection. It was determined that the current <br />financial warranty amount is adequate. <br />Hydrologic Balance: <br />The bottom of the pit was dry. There was no evidence of excessive erosion. There appeared to be no <br />significant impacts to the prevailing hydrologic balance. No mining actives have extended below the creek <br />bottom. <br />Gen. Compliance With Mine Plan: <br />The operation appeared to be following the approved mine plan. All mining activity appeared to be within the <br />approved boundary. The Operator shall continue to maintain the 100 -foot offset from the edge of East 64th Ave <br />and the affected lands. The Operator is nearing the maximum allowed disturbance of 10 acres. If the Operator <br />plans on affecting more than the maximum allowed affected area of 10 acres the Operator will need to file a <br />Technical Revision (TR) to the approved permit to increase the maximum allowed affected area. <br />Roads: <br />The access road was stable and well maintained. <br />Page 2 of 5 <br />
