April 7, 2015 C- 2009 - 087 /Peabody Sage Creek Mine JLE <br />-The berm surrounding the coal storage area and water tank has eroded and is cutting into the CH -2 channel and <br />will need to be repaired. During this inspection it was noted that Channel CH -2 is eroding downstream from the <br />berm. This channel drains to the upper sump. The channel embankment has eroded through and needs to be <br />repaired. <br />_. . <br />Channel CH -2; embankment damage Channel CH -2 Erosion <br />-Near the inlet to the Lower Sump from drainage FA -D8 where the drainage turns to drain into the sump, the west <br />side of the drainage is eroding and cutting into the slope above. PSCM will need to stabilize this area. This item <br />is still outstanding and will need to be addressed. <br />-The weir at Pond 003 is leaking and will need to be repaired. <br />- Noxious and invasive weeds were observed on the embankment of the lower sump. PSCM appears to have <br />treated this area in the past and will need to continue to monitor this area and conduct weed control as necessary. <br />REVEGETATION — Rule 4.15 <br />Vegetative Cover; Timing: <br />The solid waste disposal area was observed. Overall, the area is stable and vegetated. There is a small area where <br />a slump has occurred. Straw waddles have been installed around the slide area. Several of the waddles have been <br />broken through and need maintenance. Several rills observed where the waddles have broken through could <br />continue to erode if the waddles are not repaired. <br />Number of Partial Inspection this Fiscal Year: 8 <br />Number of Complete Inspections this Fiscal Year: 3 <br />Page 3 of 6 <br />
