OPERATORS <br />SHOULD READ THE FOLLOWING PAGES CAREFULLY IN ORDER TO ASSURE COMPLIANCE <br />WITH THE TERMS OF THE PERMIT AND APPLICABLE RULES AND REGULATIONS. If a <br />Possible Violation is indicated, you will be notified under separate cover as to when the Mined Land <br />Reclamation Board will consider possible enforcement action. <br />INSPECTION TOPIC: Hydrologic Balance <br />PROBLEM /POSSIBLE VIOLATION: Problem: The Operator failed to follow the approved Mining Plan with respect <br />to groundwater monitoring. This is a problem related to 34- 32.5- 116(4)(h) of the Colorado Revised Statutes. <br />CORRECTIVE ACTIONS: Loloff Construction, Inc. must measure the groundwater monitoring wells and <br />piezometers associated with the site by the corrective action date. The Operator must provide all available <br />monthly groundwater elevation data for surrounding groundwater wells and western Derr Pit piezometers by <br />the corrective action date. A trigger point of 2 feet change from historic groundwater levels was established as <br />part of TR -01. The trigger point elevation must be included with the groundwater elevation data. <br />CORRECTIVE ACTION DUE DATE: 4/03/15 <br />OBSERVATIONS <br />The Loloff Mine was inspected by Peter Hays with the Division of Reclamation, Mining and Safety (Division) in <br />response to a citizen complaint (CT -03) received on March 4, 2015 from Clarence Lopez on behalf of Marcos <br />Hernandez and Maria de Lourdes Palma. <br />The Hernandez property, 160 N. 1st Avenue, Greeley, CO 80631, is located northwest of the Loloff Mine. The <br />complaint letter states the source of water for the house is a domestic water well located on the property. <br />The letter states prior to November 2014 they had never experienced problems with the well and the well met <br />the demands of the tenants in the residence. The resident reported the well went dry in November 2014, <br />December 2014 and February 2015. The home owner replaced the water pump motor twice and replaced the <br />Page 1 of 5 <br />
