PERMIT #: M- 1996 -079 <br />INSPECTOR'S INITIALS: MAC <br />INSPECTION DATE: February 24, 2015 <br />OBSERVATIONS <br />The inspection was conducted by Michael Cunningham of the Division of Reclamation, Mining and Safety <br />(Division). Chester Barnard was also present for the inspection. The Barnard Quarry is located approximately 3 <br />miles north of Lyons, Colorado. The site is permitted for 9.9 acres and the post- mining land use is wildlife <br />habitat. <br />Gen. Compliance With Mine Plan: <br />The site was not active at the time of the inspection. Mining operations are generally conducted throughout <br />the warmer months. The Operator informed the Division that operations will likely resume this summer. The <br />Division informed the Operator of the requirements for Temporary Cessation. If the Operator plans to, or <br />does, temporarily cease production of the mining operation for 180 days or more, the Operator must file a <br />Notice of Temporary Cessation in writing to the Division. <br />The site was largely in the same condition as was observed during the last inspection. The Operator is <br />currently excavating in the northeast corner of the permit area. The excavation has created a highwall along <br />the east side of the permit boundary; the highwall is approximately 20' in height. The Operator is placing <br />overburden and topsoil to the south of the excavation, to be pushed back into the depression during final <br />reclamation of the site. <br />Financial Warranty: <br />The Division performed a complete bond calculation as part of the inspection and determined the current <br />financial warranty amount of $22,000.00 remains adequate to complete reclamation of the site. The Operator <br />should note the financial warranty covers the existing disturbance at the site. Any additional disturbance to <br />the site may result in an increase to the financial warranty. As noted in this report, no reclamation has been <br />performed to date. The Operator may decrease the amount of the required financial warranty be performing <br />reclamation. At any time during the life of the mine, the Operator may request the Division reduce the <br />amount of the financial warranty. The request must be made in writing and shall include an estimate of the <br />actual cost to reclaim the site based on what it would cost an independent contractor to complete <br />reclamation. <br />Signs and Markers: <br />The permit sign was posted at the site entrance and the boundary of the affected area was delineated as <br />required by Rule 3.1.12. <br />Page 2 of 3 <br />
