PERMIT #: M- 1995 -095 <br />INSPECTOR'S INITIALS: TC1 <br />INSPECTION DATE: March 18, 2015 <br />OBSERVATIONS <br />This inspection was initiated by an Acreage Release (AR) request received by the Division on March 9, 2015 <br />and deemed complete on March 18, 2015. Henry Schuler was present as the Operator for the inspection. The <br />Simla Highway Pit is a 110c gravel mine. The mine was operating at the time of the inspection. The Simla <br />Highway Pit entrance is located approximately 3 miles south of Simla off the Simla Hwy (Co. Rd 125). <br />Inspection: <br />A Permit sign was displayed as required (see Photo 1). <br />The perimeter of the permit boundary was driven and walked (where driving was impractical) to determine <br />what area could be released without future disturbance. Together with Mr. Schuler, it was determined that <br />the east half of the north end of the permit area and a narrow strip along the east side could be released <br />without future disturbance (see Figure 1). The area was measured using a Trimble Juno 3b GPS unit and <br />determined to be 1.18 acres. The point of beginning was a wood fence post near the center of the north <br />permit boundary. A loader was observed placing pushed up material from the north into a <br />dump truck (see Photo 6). <br />Records: <br />• The total permitted area is 9.9 acres per Amendment 1 approved in 2003. <br />• The previous inspection was performed on 2/14/2014. A problem was cited requiring the Operator to <br />submit either an acreage or surety reduction for completed reclamation. <br />• The anniversary date is July 8. Annual reports and fees are current through 2014. <br />• The post -mine land use is rangeland. <br />Bond: <br />The current bond is a $20,000.00 corporate surety. It has not been updated since the approval of AM -01 in <br />2003. With the resolution of the affected area addressed by the AR -01 request and this inspection, an update <br />to the financial warranty can now be addressed. The attached bond calculations are based on the 7.59 total <br />affected area (9.9 acres from Amendment 1, minus 1.18 acres to be released with the recommended approval <br />of AR -01, minus 1.13 acres estimated for the sediment pond that will not be topsoiled or seeded). The revised <br />bond estimate is $28,540.00. Please review the attached bond calculations and address anv questions to the <br />Division within 30 days of the date of this report. A surety increase letter will sent to the Operator at that <br />time. <br />Summary and Recommendations: <br />1. The Division will recommend approval of AR -01. <br />2.
