PERMIT #: M- 2005 -069 <br />INSPECTOR'S INITIALS: SJM <br />INSPECTION DATE: January 27, 2015 <br />OBSERVATIONS <br />This was a monitoring inspection of the West Fork Pit; M- 2005 -069 operated by Encana Oil and Gas, Inc. This <br />site is located than 15 miles north northwest of Parachute, Colorado in Garfield County. I, Stephanie Mitchell <br />of the Colorado Division of Reclamation, Mining and Safety conducted the inspection. Mr. Jason Eckman and <br />Rod Cogburn of Encana Oil and Gas accompanied me on the inspection. <br />This is an 18.4 acre 112c mining operation for sand and gravel. A mine identification sign was in place at the <br />entrance to the mine site. There were no permit boundary markers present and has been noted as a problem <br />at the beginning of this inspection report. <br />Topsoil and material stockpiles appear to be stable. <br />No noxious weeds were observed on site. <br />The Operator mentioned that they may want to change the post mining land use. The permit states that the <br />post mining land use is Rangeland. If the Operator wishes to pursue a change in the post mining land use, an <br />Amendment to the permit will need to be submitted.
