PERMIT #: M- 1980 -244 <br />INSPECTOR'S INITIALS: TC1 <br />INSPECTION DATE: March 11, 2015 <br />Summary: <br />1) The Division has determined that a Technical Revision is not required for the stemming material <br />crushing operation performed by an outside contractor. There is no significant impact to the <br />reclamation plan or the financial warranty. The Division asks that CC &V keep us apprised of where <br />stemming material crushing operations are to take place in the future. <br />2) CC &V agreed to submit a Technical Revision to address the SGVLF pumpback system design <br />modifications, design specifications (including pipes and geosynthetics), and monitoring plans for the <br />concrete ponds. System design specifics of interest to the Division are: pipe pressure capacity, <br />overflow sump operation, discharge location, post- mining reclamation, and impacts to the financial <br />warranty, if any. <br />3) The Division is considering the need for additional water level monitoring, outside the potential cone <br />of depression resulting from the PSSA pumps, in both the AGVLF Phase V PSSA and the SGVLF PSSA. <br />The Division will inform CC &V of its decision. <br />4) Mr. Chris Hanks (CC &V) emailed the Division on March 12, 2015 to inform us that the LVCS reading log <br />books are stored in the Process Operations Superintendent's office in the Carlton ADR. <br />5) In response to a follow up inquiry from Mr. Hanks, the Division stated they have no objections to <br />removing Grassy Valley monitoring station GV -01. The Division will inform CC &V what actions need to <br />be taken regarding the lack of data from the Grassy Valley monitoring stations. <br />6) The Division will endeavor to coordinate process facility inspections one or two days in advance of the <br />next inspection. <br />PHOTOGRAPHS <br />Photo 1.
