Reclamation Permit Amendment Application Package
Dear Mr. Wayland,
On March 4, 2015, the Division of Reclamation, Mining and Safety (Division) received your 112
Construction Materials Reclamation Permit Amendment Application package for the Brownwood Pit,
File No. M -1979 -059. Preliminary review of the application package determined that the following items
must be received before the Division can consider your application as being submitted and technical
review can begin:
Application Form
1. The application form states the Applicant's name is `Coulson Excavating Co., Inc.'. However, the
company name is listed as `Coulson Excavating Company, Inc.' with the Colorado Secretary of State.
The Applicant/company name must be listed throughout the application form exactly as it is
registered with the Secretary of State. Please revise the applicant/company name of Pages 1, 3 and 8
of the application form and submit replacement copies to the Division.
2. The Applicant did not answer questions No. 6 and No. 7 on the second page of the application form.
Please revise this page and submit a replacement copy to the Division.
3. The Certification section (page 8) of the application form was filled out incorrectly. The
`Applicant/Operator or Company Name' field should contain the name of the applicant as it is listed
on the first page of the application; see additional comments under No. 1. Please revise the
Certification section and submit a replacement copy to the Division.
