OPERATORS <br />SHOULD READ THE FOLLOWING PAGES CAREFULLY IN ORDER TO ASSURE COMPLIANCE <br />WITH THE TERMS OF THE PERMIT AND APPLICABLE RULES AND REGULATIONS. If a <br />Possible Violation is indicated, you will be notified under separate cover as to when the Mined Land <br />Reclamation Board will consider possible enforcement action. <br />INSPECTION TOPIC: Availability Of Records <br />PROBLEM /POSSIBLE VIOLATION: Problem: No Geotechnical Stability Exhibit (Rule 6.5) is on file with the <br />Division. <br />CORRECTIVE ACTIONS: The Operator shall submit a Technical Revision, with the required $216 fee to address: <br />Rule 6.5(1) - provide a geotechnical evaluation of all geologic hazards that have the potential to <br />affect existing and proposed highwalls; Rule 6.5(2) - provide engineering stability analyses for proposed final <br />reclaimed highwalls, and Rule 6.5(3) - demonstrate through appropriate geotechnical and stability analyses that <br />off -site areas will be protected with appropriate factors of safety incorporated into the analysis. <br />CORRECTIVE ACTION DUE DATE: 5/11/15 <br />Page 1 of 8 <br />
