PERMIT #: M- 1998 -059 <br />INSPECTOR'S INITIALS: ERR <br />INSPECTION DATE: November 3, 2014 <br />OBSERVATIONS <br />The Division of Reclamation, Mining and Safety (Division) conducted an inspection of the Sunny Dale Farm Pit (Permit <br />File No. M- 1998 -059), a Construction Material Regular 112c Operation Reclamation Permit with 45 permitted acres and <br />an approved post - mining land use of Pastureland. The mine is located approximately 8.5 miles west of Holyoke, <br />Colorado. Elliott Russell, with the Division, inspected the site while Mike Koellner, representing the operator, <br />accompanied it. This inspection was carried out as a part of the Division's normal monitoring program. <br />At 15:00, the Division met Mr. Koellner at the mine site entrance, located %z mile east of CR 21, on CR 22. At the time of <br />the inspection it was cold, raining, and the ground was wet. The mine site identification sign was observed at the entrance <br />to the site and was in compliance with Rule 3.1.12(1). The permit boundary was delineated by fences and t- posts. There <br />was no equipment on site at the time of the inspection. <br />The site is located along the South Fork of Frenchman Creek; this ephemeral channel was not flowing at the time of the <br />inspection, but the site did contain some standing water from recent rain events. The permitted area consists of two main <br />excavation areas; one on the western half of the permit, which is the upstream side of the creek, and another on the eastern <br />half of the permit, downstream side. The eastern portion was accessed by a separate entrance along CR 23, just south of <br />CR 22. Both access roads had been seeded and were no longer in use. The site appeared to be adequately vegetated and <br />showed no signs of erosion. The Division observed evidence that the area was recently being grazed by livestock, which <br />corresponds to the post - mining land use. <br />A small concrete ruble pile, approximately 1 C.Y., was observed in the northeast corner of the permit on the southern <br />bank of the creek. This pile will need to be removed prior to final release of the site. The footprint of this pile should be <br />broadcast with the reclamation seed mixture to help prevent the establishment of unwanted weed species. <br />Once this pile has been removed from the site, the operator may request the Division for final bond release. Per Rule <br />4.17.2, the Division will conduct a final inspection to evaluate the success of the reclamation efforts and determine if the <br />operator has complied with all applicable requirements. <br />Inspection Contact Address <br />Mike Koellner <br />L & L Ready Mix <br />P.O.
