This permit was originally <br />listed under Siegrist Construction Company. A permit amendment was approved by the <br />Division as Amendment 01 on September 25, 2006, and a Succession of Operators <br />Approval was granted by the Division on October 25, 2006. The new operator is now <br />Bayshore, LLC (Formerly Carma Bayshore, LLC). Recorded of the corporate name <br />change has been filed with the Division. <br />The anniversary of the original permit is December 10. We understand that an Annual <br />Report is due each year at that time. The report should address any activities that have <br />occurred within the permit area. A check for the permit's annual fee in the amount of $791 <br />is attached. No changes to the map on file are necessary as no new disturbances or <br />reclamation has occurred during this year. <br />A revised grading and reclamation plan was submitted and approved by the Division in <br />2012. Work for the final shape grading and soil enrichments where well underway in <br />September 2013. The historic floods during September breached the embankment of the <br />facility. Our team has been working with FEMA, Army Corps of Engineers, and Weld <br />County to repair the damage.
