Activities were limited <br />to maintenance of slopes and erosion control measures. No major erosion was observed <br />on or near the area of operation. Permit boundary signs are intact and the overall <br />condition of the site was acceptable. Please find an attached site map I check for an <br />annual fee of $791.00 and a second check for penalty of payment for another $791.00. <br />We appreciate your time and efforts.
