PERMIT #: M- 1980 -244 <br />INSPECTOR'S INITIALS: TCI <br />INSPECTION DATE: January 7, 2015 <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the site on January 7, 2015. Chris Hanks, represented the <br />Operator during various parts of the inspection. Tim Cazier and Russell Elliott represented the Division. The <br />focus of this inspection was to observe water levels, construction status for the Squaw Gulch VLF, and other <br />ongoing construction projects. <br />Pre - Inspection Meeting: <br />The Division attended a meeting with representatives from CC &V (Timm Comer, Jeff Winterton, Chris Hanks, <br />and Marc Tidquist) to discuss water management in the Arequa Gulch VLF, final design changes to the topsoil <br />stockpile /visual berm adjacent to Poverty Gulch, and the pending Squaw Gulch PSSA Construction Certification <br />adequacy responses. Mr. Winterton explained that water levels in the AGVLF Phase I PSSA were above the <br />80% capacity and how the mine was working to correct the situation. The Division formally requested <br />information on the four PSSAs in the AGVLF. In regards to the visual berm, the Division confirmed that any <br />increase in the affected permit area requires an amendment. CC &V indicated they would revisit the visual <br />berm designs to comply with the existing approved amendment 10 permit. The adequacy responses were <br />discussed with Chris Hanks, Ron DiDonato and Scott Redabaugh at the Process Office later during the <br />inspection. The Division agreed to the approach presented in revised drawings for the SGVLF PSSA adequacy <br />responses. Document submissions for both the AGVLF PSSAs and adequacy responses were submitted to the <br />Division subsequent to the inspection. On January 9, 2015 Mr. Comer confirmed via telephone that water <br />levels in the Phase I PSSA had dropped to below the 80% capacity level and stayed there. <br />Inspection: <br />A permit sign was observed at the ADR entrance (see Photo 1). <br />Construction: <br />In the Squaw Gulch VLF, the Division observed: <br />• Placement of structural fill and spent ore for the purpose of building an access ramp to the PSSA risers <br />in the southwest end of the SGVLF PSSA. (see Photo 2); <br />• SGVLF liner construction was shut down for the season; <br />In the Squaw Gulch VLF, the Division observed: <br />• Construction progress on stormwater ponds EMP -09 a, b, c, and d. (see Photos 3, 4, and 5). <br />Water levels: The inspection continued as the Division visited each of the high and low solution collection <br />system transducers and recorded water level values. The recording sheet is included as Attachment A, and <br />the values are summarized below in the Transducer Readings. <br />Page 2 of 6 <br />
