OPERATORS <br />SHOULD READ THE FOLLOWING PAGES CAREFULLY IN ORDER TO ASSURE COMPLIANCE <br />WITH THE TERMS OF THE PERMIT AND APPLICABLE RULES AND REGULATIONS. If a <br />Possible Violation is indicated, you will be notified under separate cover as to when the Mined Land <br />Reclamation Board will consider possible enforcement action. <br />INSPECTION TOPIC: Signs & Markers <br />PROBLEM /POSSIBLE VIOLATION: Problem: The Permit was transferred from Robinson Brick to General Shale in <br />2008. The Permit Signs still lists Robinson Brick as the Operator. <br />CORRECTIVE ACTIONS: Correct the Operator name to General Shale Brick, Inc. on the Permit Sign(s) and submit <br />photographic documentation demonstrating the correction by March 11, 2015. <br />CORRECTIVE ACTION DUE DATE: 3/11/15 <br />Page 1 of 7 <br />
