January 13, 2015 C- 1981- 008/New Horizon Mine BFB <br />The Pond 11 contained water and was discharging. The discharge structure was free and clear of obstructions. <br />Water was being pumped from the trench drain into Pond 11. The rodent holes in the Pond 11 embankment were <br />repaired and backfilled (photo 2). The gullies in the northeast corner of the embankment were also repaired. <br />Culverts 108, 117, 117A, 118, 120 and 121 were clean. <br />Ditch C18 was clean. The cattails had been removed and the ditch cross section had been restored on the west end <br />(photo 3). The entire length of the ditch was not inspected because the area had been electric fenced, cattle were in <br />the area and the ground was very muddy. The ditch will be inspected more thoroughly when the conditions are <br />better. <br />OTHER (SPECIFY): <br />A telephone conference was held at the New Horizon mine office on January 15, 2015. In attendance at <br />the meeting were Frank Ferris, Trevor Ragsdale, Rick Wolney and Tony Tennyson (phone -in) representing <br />Western -Fuels and Dan Hernandez (phone -in) and Brock Bowles of CDRMS. The items discussed were: <br />1. WFC contacted the ditch company to determine the amount of water shares that belong to the Burbridge's. <br />WFC feels that the designed water system is adequate to deliver the proper amount of shares to the Burbridge's <br />property. WFC was going to contact Mr. Burbridge to explain the piping system and demonstrate that it is <br />adequate to supply his shares water. <br />2.WFC is still looking into the water quality of the water wells on the Burbridge property. <br />3. The land around Pond 8 on the Burbrige property has been Phase II bond released except for approximately 3 <br />acres on the northwest corner. This area can be designated for small area exemption. <br />4. The house east of the NH2 mine office needs to be incorporated into the permit and WFC was unclear about <br />creating sediment control for the area. WFC can do a SEDCAD demonstrating that the current landscaping is <br />adequate to control erosion. If the landscaping is found to be adequate, then no sediment control structures need to <br />be installed. <br />5. Baseline data is not needed for the house if the pre- mining land use is residential, the post- mining land use is to <br />remain residential and no major alterations to the property are proposed. <br />6. WFC informed DRMS that ditch C -18 has been cleaned and restored to its designed cross section. <br />7. It is unclear who is responsible for maintaining the culverts under the BB and 2700 Roads. The roads were <br />Phase III released in SL -13. WFC will contact the county road department to learn their policies and Brock will <br />look into if the mine related disturbances above the culverts have been bond released. <br />ROADS — Rule 4.03 <br />Construction 4.03.1(3)/4.03.2(3), Drainage 4.03.1(4)/4.03.2(4), Surfacing and Maintenance4.03.1(5) and <br />(6)/4.03.2(5) and (6), Reclamation 4.03.1(7)/4.03.2(7): <br />The BB Road west of the 2700 Road was very muddy.
