Maps C -1 and C-4 as submitted do not include the portion of the affected area north of 132 "d <br />Ave (please see the enclosed index map from AM -01). Please revise Maps C -1 and C-4 to include the <br />affected area north of 132 "d Ave. <br />EXHIBIT F - Reclamation Plan May (Rule 6.4.6): <br />4) Please provide a Reclamation Plan Map(s) that illustrates how all of the affected land will be reclaimed (this <br />will need to include what is currently permitted and what is being added, please see the attached <br />Reclamation Plan Map from AM -01). The map(s) of the affected area, shall indicate the following: <br />a. The expected physical appearance of the area of the affected land, correlated to the proposed mining <br />and reclamation timetables. The map must show proposed topography of the area with contour <br />lines of sufficient detail to portray the direction and rate of slope of all reclaimed lands; and <br />b. Portrayal of the proposed final land use for each portion of the affected lands. <br />EXHIBIT M - Other Permits and Licenses (Rule 6.4.13): <br />5) Please provide a list of permits that were required for the construction of the inlet/outlet structure. Were <br />any of these permits required for the construction: effluent discharge permits, disposal of dredge and fill <br />material (404) permits, well permits, highway access permits, county zoning and land use permits, and <br />city zoning and land use permits. <br />EXHIBIT N - Source of Legal Right to Enter (Rule 6.4.14): <br />6) Please demonstrate that the Applicant/Operator can enter the property owned by Henderson Aggregates, or <br />please demonstrate the terms of the easement between Henderson Aggregates and United Water Sanitation <br />District grants the Applicant/Operator legal right to enter on the property owned by Henderson Aggregates. <br />7) Please demonstrate that the Applicant/Operator can enter the property owned by Adams County for the <br />portion of the inlet/outlet structure underneath the Brighton road right -of -way. <br />EXHIBIT R - Proof of Filing with County Clerk and Recorder (Rule 6.4.18): <br />8) The submittal for this exhibit did not demonstrate that a copy of the AM -03 application was placed with the <br />local County Clerk and Recorder for public review, as required by Rule 1.6.2(1)(c). Please submit an <br />affidavit or receipt indicating the date on which the application was placed with the local County Clerk and <br />Recorder for public review, as required by Rule 1.6.2(1)(c). <br />9) Please note that any changes or additions to the application on file in our office must also be reflected in <br />the public review copy, which was placed with the Adams County Clerk and Recorder. <br />
