(Form 2 - (Public File) <br />The road will be constructed from natural earth excavated on site for grading along with estimated 40 yards of <br />pit run and road base material to be hauled in by truck. Top soil will be removed and stock piled 50 ft from the <br />NW comer of the drill pad for future reclamation. Road cuts will be minimal (not exceeding 4 ft) and can be <br />handled by a dozer and loader. Water diversion channels will be dug on each side of road easement to <br />connect with the 2 large culverts placed 200 ft from road entrance. <br />Disturbances include removal of top soil and removal of brush. No timber removal is necessary and no large <br />pits will be excavated. Brush will be disposed of at the county land fill. <br />4. Describe how drill holes will be plugged (refer to Rule 5.4 of the <br />Rules for required abandonment procedures): <br />All drill holes will be plugged with on -site drilling equipment immediately after completion of each hole. The holes <br />will be plugged with Neat Cement Grout. The cement will be placed through the drill pipe and circulated from the <br />bottom of drill hole to the top of the hole, as drill pipe is removed from the hole. This is to prevent vertical <br />movement of ground water, if encountered during drilling operation. Plugging entire length of all drill holes will be <br />done regardless of whether dry or wet holes are encountered as stipulated by all State of Colorado standards for <br />drill hole plugging. Casing will be removed from the top of hole prior to closing the drill hole. The completed drill <br />holes normally can be plugged during the next day of operations. <br />5. Describe how portals, adits, shafts, ponds, excavations, or other <br />disturbances will be reclaimed (refer to Rule 3 and Rule 5 for specific <br />reclamation performance standards). You may wish to contact the Division <br />for closure specifications. <br />6. Describe how roads will be reclaimed or returned to their pre - <br />prospecting (or better) condition: <br />Drill site pad and road minimize disturbance to vegetation. Re- vegetation of ground along road and drill pad will be <br />carried out, using a certified, noxious /weed -free seed mix preapproved by USFS. <br />All drill holes will be plugged as drill casing and drill pipe will be removed from a hole with on -site drilling equipment <br />immediately after completion of each hole and plugged in accordance with Colorado State Standards for well <br />closure in accordance with well abandonment regulations (CO Reclamation Requirements 34 -32 -113). The holes <br />will be plugged with Neat Cement Grout. It is assumed that each drill hole would take an average of 5 -10 days to <br />close. The longest any hole would remain open should be no more than 3 days. <br />Reclamation of sump will be handled at the end of drilling program. Reclamation will involve removing liner material <br />and filling the sump back to original condition. Material dug out to construct the drill pad will be restored or replaced <br />on top of the drill pad. The top will be level with the adjacent overburden. The drill pad area will be seeded with a <br />certified seed mix pre approved by the USFS. <br />No drill cuttings from the sump will come in contact with ground or surface water. <br />The road will be returned to preexisting condition after drilling program is completed including removal of all <br />culverts, gate and fencing materials around pad and signs. Straw bales and waddles will be installed in areas that <br />would be susceptible to erosion and sedimentation. <br />
