2.04.13 Annual Reclamation Report <br />(1) By February 15th of each year the applicant will file an annual reclamation report <br />covering the previous calendar year for all areas disturbed under bond. The <br />report will include text, discussion and maps which address: <br />(1)(a) The name and address of the permittee and permit number; <br />(1)(b) Location and number of acres disturbed during that year; <br />(1)(c) Location and number of acres backfilled and graded during that year; <br />(1)(d) Location and number of acres topsoiled during that year; <br />(1)(e) The species, location and number of acres of vegetation planted during that year, <br />including any augmented seeding or cultural practices; and <br />(1)(f) Location, number of acres and date of planting for all previously revegetated <br />areas. <br />Permit Application 2.04 -41 04/11 <br />
