Preliminary review of the information received determined the <br />following items must be received before the Division can consider the application as being <br />submitted and technical review can begin: <br />Application Form <br />1. The Applicant indicates on Section #3 -Permitted acreage the site is permitted for 42.7 acres <br />with the addition of 5.1 acres for a total acreage in the permit area of 47.8 acres for the <br />amendment application. The site is currently permitted for 55.17 permit acres with 42.7 <br />affected acres. Please review the existing and proposed additional acreage amounts and <br />revise the application form as needed. <br />2. Please revise Section #16 on the application form to indicate the correct permitted and <br />affected acres as required by Question #1 above. <br />3. The Division requires the application list the exact company name registered with the <br />Colorado Secretary of State office. The Applicant's name in the notary section on Page 8 - <br />Certification of the amendment application form does not match the registered company <br />name. Please revise the notary section of the certification page to indicate "Loloff <br />Construction, Inc." <br />4. The Division requires the Applicant to provide documentation giving the signer the authority <br />to sign on behalf of the company.
