PERMIT #: M- 1980 -244 <br />INSPECTOR'S INITIALS: TC1 <br />INSPECTION DATE: November 5, 2014 <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the site on November 5, 2014. Chris Hanks, Marc Tidquist, <br />Ron DiDonato and Scott Redabaugh represented the Operator during various parts of the inspection. Tim <br />Cazier and Amy Eschberger represented the Division. The focus of this inspection was to observe water <br />levels, and ongoing construction for the Squaw Gulch VLF. <br />Meeting: <br />The Division attended a meeting with representatives from CC &V, Ames Construction and Amec to discuss the <br />pending certification report for the completed Squaw Gulch VLF Pregnant Solution Storage Area (PSSA) <br />construction. <br />Inspection: <br />Construction: In the Squaw Gulch VLF, the Division observed: <br />• Placement of the third 28 -inch diameter, perforated, primary high volume solution collection system <br />HDPE pipe (see Photos 1 and 2), and butt fusion pipe welder (see Photo 3); <br />• Top of primary geomembrane liner on the floor of the PSSA (see Photo 4); <br />• Tie -in of the secondary 24 -inch diameter, perforated, primary high volume solution collection system <br />PCPE pipe (see Photo 5) <br />• Placement of the 4 -inch diameter, perforated, tertiary high volume solution collection system PCPE <br />pipe (see Photo 5); and <br />• Installed PSSA louvered riser pipes (see Photos 6A and 6B); <br />Reclamation plan: Mr. Hanks pointed out the erosion gullies on the Hwy 67 /SGVLF toe berm and explained <br />the mines intent to riprap the downchutes from the CDOT drainage pipes under Hwy 67. He indicated where <br />the diversion channels were being constructed around the underdrain ponds (see Photo 7). <br />Water levels: The inspection continued as the Division visited each of the high and low solution collection <br />system transducers and recorded water level values.
